GENERAL SERVICE TERMS

9. Services Provided

Divine Wellness and Primary Care provides [INSERT FULL SERVICE LIST — e.g., primary care, IV therapy, peptide therapy, etc.]. Service frequency, eligibility, and pricing are agreed upon at scheduling or consultation.

10. Customer Responsibilities

⚠️ This entire section is pet-service-specific (yard access, gates, aggressive pets) and needs to be fully rewritten for a medical practice. Typical replacements include: providing accurate and complete health history, arriving on time or per the practice’s late/no-show policy, disclosing medications/allergies/conditions relevant to treatment (especially important for IV and peptide therapy), following pre/post-treatment instructions, and informed consent requirements. This should be drafted with input from the clinic’s providers and reviewed for compliance.

11. Scheduling

Appointment availability may vary due to holidays, provider schedules, or unforeseen circumstances. We reserve the right to adjust schedules as needed.

12. Payments

Payment is due according to the billing terms selected at scheduling. Failure to maintain valid payment may result in suspension or cancellation of service.

13. Cancellation

Customers may cancel service with reasonable notice prior to the next billing cycle or appointment. Same-day cancellations or no-shows may be charged as a completed visit. [Confirm the clinic’s actual no-show/late-cancellation policy and fee, if any.]

14. Limitation of Liability

⚠️ This entire section is pet-service-specific (yard damage, pet escape, fencing) and does not apply to a medical practice. A medical practice’s limitation-of-liability language needs to be drafted carefully alongside informed consent language, and typically addresses things like: individual treatment outcomes are not guaranteed, risks associated with IV/peptide therapy specifically, the practice is not a substitute for emergency care (with a clear “call 911 or go to the nearest ER” instruction), and any other disclaimers appropriate to the services offered. This should not be adapted from a pet-service template — it needs to be written from scratch with legal/clinical input.

15. Modifications to Terms

We may update these Terms at any time. Continued use of services constitutes acceptance of updated Terms.
